- The Medicare Diabetes Prevention Program (MDPP) is one of the free health programs available for seniors on Medicare. It’s a Diabetes Prevention Program (DPP), which has been shown to prevent or delay the onset of type 2 diabetes through healthy lifestyle changes, but it’s for seniors.

A diagnosis of prediabetes can be startling. If your doctor told you that you have prediabetes or are at risk for diabetes, or you got a lab test showing higher than normal blood sugar levels or A1C, it’s normal to feel overwhelmed. But it’s common to have high A1C - over half of seniors have blood sugar in the prediabetes range - and Medicare offers coverage for programs to manage it.
The options in 2026 include the Medicare Diabetes Prevention Program (MDPP). For eligible participants, the Centers for Medicare and Medicaid (CMS) the MDPP is available at no cost to you. Here’s why prediabetes is a wake-up call that can empower you to take charge of your health, why one of the free diabetes prevention programs in Medicare can have such a big impact, and what’s covered in the MDPP.
You don’t have to face prediabetes alone, make major life changes, or pay a lot of money to make an impact. With MDPP being a free Medicare program for seniors who qualify, Lark MDPP’s digital health coach may be a solution for you that’s covered and completely convenient through your phone.
Why Is Diabetes Prevention So Important?
It’s so important to provide a free diabetes program for seniors because of how widespread prediabetes is, how fast it can convert to diabetes, and also because of how much of an impact lifestyle changes can have in managing blood sugar.
The National Diabetes Statistics Report says that over half of seniors have blood sugar that puts them in the range for prediabetes - higher than normal ranges, but lower than in diabetes. Having prediabetes means you already have insulin resistance, or your body doesn’t respond as well to insulin as it used to. It also means you may be at higher risk for cardiovascular disease.
Numbers vary, and individual risk varies, but the Standards of Care 2026 from the American Diabetes Association (ADA) state that about 5-10% of people with prediabetes will be diagnosed with diabetes within a year. About 2 in 3 people with prediabetes will develop diabetes within their lifetimes.
Sobering? Yes. Prediabetes is serious. But wait…there’s good news, too!
The first piece of good news is that there’s a lot you can do to lower your risk for developing diabetes. It involves small, healthy changes to your lifestyle - not a major overhaul or anything drastic, just small consistent changes in areas like what you eat, how active you are, and how you manage stress and sleep. Examples include ordering salmon instead of steak, or adding a 10-minute walk after dinner.
Research shows that these changes can lower risk for diabetes. In a landmark study, seniors who participated in a lifestyle change program lowered risk for diabetes by over 70%. The main goals were modest: losing 5-7% of body weight, and achieving at least 150 minutes per week of moderate to vigorous intensity exercise.
More good news is that support is available with programs for seniors to lower blood sugar. You don’t have to do this alone. In addition, with coverage from CMS, the Medicare Diabetes Prevention Program is available at no cost to eligible participants, even without a doctor’s referral.

How to Qualify for a Free Prediabetes Program for Seniors
Free health programs for seniors on Medicare can be complicated sometimes, but that’s not so with the MDPP! You don’t need a doctor’s referral, and there are no hidden costs. Here are the simple criteria for how to qualify for MDPP.
- Insurance: You must be enrolled in Medicare Part B or Medicare Advantage (Part C)
- Blood Tests: You need to have a lab result from within the past 12 months. You can check the qualifying values in the table below.
- Weight: You must have a body mass index of at least 25, putting you in the overweight or obese category. For Asian Americans, the qualifying BMI value is at least 23 kg/m2.
You’re not eligible for MDPP as a covered benefit in Medicare if you have either of these.
- A previous diagnosis of type 1 or type 2 diabetes. (It’s okay if you had gestational diabetes - diabetes during pregnancy).
- Living with end-stage renal disease (ESRD).
New in 2026 is more good news if you are a senior with diabetes: there’s no more limit of one MDPP enrollment per lifetime. That means that you’re not disqualified from MDPP now even if you were in a diabetes prevention program previously.
What’s Included in the MDPP - And Why A Free Diabetes Prevention Program on Medicare Matters
The CMS covers the entire MDPP - curriculum, coaching, goal-setting, and everything. There’s no cost to eligible participants. You don’t need to worry about a co-pay, a deductible, or any hidden costs. All you have to do is qualify and enroll.
Why does it matter? It’s important for enrollment to be easy and at no cost to you because the program works. When you make healthy changes, when you lose extra weight, and when you increase your physical activity - you’re making a real difference. You’re not just lowering your risk for diabetes, but you’re supporting heart health, kidney health, and more. Making lifestyle changes with the full support of a comprehensive program is a major step forward. It matters.
